Permission Forms

Permission is a very important part of any filming project you need to make sure that all partys have given permission to be filmed and that they are completely comfortable with that. whilst you can just get permission by getting your subject to look into the camera directly and give verbal permission it is sometimes preferred if the subject signs a set permission form. 

For this project i have avoided filming minors, as I don't want the project to fall into a immature light (as my target audience is young adults from 18-25) and any inclusion of minors might drive away my primary target. However if I were to have a interview with a minor or do any other sort of filming including a minor I would have to have full written consent from the parent and guardian and the agreement from the minor themselves.

If permissions are not fully given there can be dire consequences.
